Admin or therapist adds a standardized assessment score to a client's record. The scoring framework carries through: domain, subdomain, composite, standard scores, percentile ranks, age-equivalent values, whatever the instrument produces. Those scores surface on the Therapy Record's evaluation card, feed the POC generator alongside the guided evaluation data, and appear as quoted evidence in AI-generated reports.
Like guided evaluations, standardized assessments are structured data, not prose. That means when a report merges data sources, 'evaluation, standardized assessments, session notes', the AI composes from the scored results, not from a free-text field where you typed the scores. Age-equivalent, percentile, and standard-score values render appropriately in medical and educational reports alike.

Each instrument carries its own domain hierarchy, score types (raw, standard, scaled, percentile, age-equivalent), and interpretation bands. Scoring input validates against the instrument's structure, no out-of-range entries, no mismatched score types.
POC generator reads guided-eval + assessment data together when identifying below-competence domains. AI report composition quotes assessment results appropriately for the audience, standard-score language for medical, percentile + age-equivalent for educational.
