Reports compose themselves from measured data, not from re-reading your own narrative notes. When you create a report, you select which data sources to merge, evaluation results, session notes, appointment history, standardized assessments, integrated forms, and the audience (medical or educational). The report comes out appropriate for both the data and the reader.
A medical report speaks to insurance reviewers, medical necessity, skilled intervention, functional deficits, measurable clinical change. An educational report speaks to IEP teams and school-based providers, educational necessity, participation, strengths-based framing, access to curriculum. Across both, the report is built from scores: if the data shows a client improved from 15% to 45% accuracy, the report says so, because the numbers are there. If data is limited, the report says so. If progress is ambiguous, the report doesn't overclaim.
