Overview

Reports compose themselves from measured data, not from re-reading your own narrative notes. When you create a report, you select which data sources to merge, evaluation results, session notes, appointment history, standardized assessments, integrated forms, and the audience (medical or educational). The report comes out appropriate for both the data and the reader.

A medical report speaks to insurance reviewers, medical necessity, skilled intervention, functional deficits, measurable clinical change. An educational report speaks to IEP teams and school-based providers, educational necessity, participation, strengths-based framing, access to curriculum. Across both, the report is built from scores: if the data shows a client improved from 15% to 45% accuracy, the report says so, because the numbers are there. If data is limited, the report says so. If progress is ambiguous, the report doesn't overclaim.

The AI writes the language, but the findings come from the structured data underneath, evaluation scores, session GPIs, assessment results, form submissions, all traceable, all defensible, all composed for the audience. That's the difference between AI that generates plausible-sounding clinical prose and a reporting system that generates accurate clinical prose because it's working from data that's already been measured.

Medical reports include a medical necessity statement grounded in the measured data. Educational reports include an educational necessity statement framed around access, participation, and engagement. The appropriate statement composes automatically from the audience selection.

Quarterly summary integrated into the Therapy Record.

Quarterly summary card renders domain progress tiles plus AI-generated summary text. Visible inline on the client's Therapy Record without opening a report. Full progress reports generate on demand from the same underlying data.

Data limitations surface in the report.

If session data is sparse, the report includes a caveat. If progress is ambiguous, the report reflects that. The system is designed to not overclaim, because overclaimed reports are the ones that get denied.
