When you run a guided evaluation in MyTherapyWizard, the evaluation itself knows which items measure which domain, visual perception, motor planning, receptive language, safe mobility, executive function, whatever the instrument is designed to assess. You're not filling in prose and hoping the narrative reads well later. You're capturing scored performance that the rest of the system, plan of care, session notes, progress reports, can read and reason from.
When the evaluation is complete, you don't just have a report stuck in a PDF. You have a profile. You know which domains the client performs within expected range, which domains fall below competence, and where the baseline sits within each affected domain. That profile is what the POC generator reads from, what the goal bank is sequenced against, and what progress reports compare session trajectories back to.

Each evaluation template carries a domain hierarchy (composite → domain → subdomain) with scoring rules per item. Scores aggregate automatically. Below-competence domains flag for POC prioritization.
Templates are org-configurable with a drag-and-drop builder. Item order, scoring ranges, and domain mapping are editable without a developer. Platform templates available as starting points.
The evaluation linked to the active POC always displays first on the therapist's Therapy Record, visually distinct (teal border, teal-light background, Active eval chip), above the POC card. In-progress evaluations show a Continue button.
Client Trends analytics pulls composite, domain, and subdomain scores across the caseload, filterable by OT_status, therapist, grade, age, diagnosis, district, school. Tabular, exportable to CSV / Excel.
